Enter a Laboratory No to identify the laboratory.
Enter a Laboratory Name to describe the laboratory.
Select the Laboratory Type to identify the type of laboratory. Available options are: Food Services, Health Services Diagnostic, Private Diagnostic, Quality Control and State Diagnostic.
